El Deah Support Coordination Services supports NDIS participants to understand their plans, connect with the right providers, and access the services they need to achieve their goals. We work alongside participants, families, and carers to ensure supports are coordinated effectively and tailored to each person’s needs.
Our services include Support Connection, Support Coordination, Specialist Support Coordination, and Positive Behaviour Support. We assist with behaviour support assessments, development and implementation of Behaviour Support Plans, community participation, accommodation and living supports, and linking participants with NDIS providers.
Our focus is to empower participants through choice and control while building capacity for greater independence and meaningful participation in their communities.
